When you first meet Nocturnal you get several toys equipped and have access to swap for many others using the MCM Toys & Tats tab, even when you don't have the key.
Others are obtained later in the story, including quests such as Tales & Tallows, Maiden Liberation, and Foot Fetish. These generally cannot be removed or swapped until the quest is completed.
